术后正负性情绪对女性乳腺癌患者预后的影响

摘要:

目的:探讨术后正负性情绪对女性乳腺癌患者预后的影响。方法:使用正负性情绪量表对277例女性乳腺癌患者术后一周的正、负性情绪进行评定,并收集患者一般人口学资料和临床病理资料。5年后,采用电话随访了解患者生存情况。采用Kaplan-Meier法计算生存率,采用Log-rank检验及Cox回归分析分别对分类变量和连续变量进行预后因素分析。结果:乳腺癌患者5年生存率为75.45%。淋巴结分期、长期居住地、正性情绪、负性情绪均为乳腺癌预后的影响因素(均P<0.001)。但控制淋巴结分期和长期居住地后,正性情绪仍显著影响患者预后(RR=0.756;95% CI 0.701-0.815;P<0.001),而负性情绪对预后的影响不再显著。结论:术后情绪影响乳腺癌预后,其中正性情绪对预后的影响比负性情绪更加重要。

Objective:To examine whether and how post-surgery positive affect and negative affect influence the prog-nosis of women with breast cancer. Methods:In total, 277 women with breast cancer completed a form for socio-demo-graphic and clinical information as well as the Chinese version of Positive and Negative Affect Scale. Kaplan-Meier method was conducted for survival analysis. Log-rank test and Cox regression analysis were conducted respectively for categorical variable and continuous variable. Results:The five-year survival rate for patients was 75.45%. The results of Log-rank test showed that there were significant differences between survival and non-survival patients in AJCC nodes pathologic sta-tus and long-term residence place(all P<0.01). The results of Cox regression analysis showed that post-surgery positive af-fect significantly predicted prognosis of women with breast cancer after controlling significant prognostic factors in univari-ate analysis(RR=0.756; 95%CI 0.701-0.815; P<0.001) while the influence of negative affect was not significant. Conclusion:Post-surgery emotion influences the prognosis of women with breast cancer. Notably, the influence of positive affect on prognosis of breast cancer is greater in comparison with negative affect.
