基于功能磁共振的单相与双相抑郁障碍神经生物机制研究进展

摘要:

单、双相抑郁障碍患者临床均以情绪低落、兴趣缺乏、乐趣丧失等症状为特征,常常难以准确区分.本文综述了单、双相抑郁障碍功能磁共振比较研究,发现单、双相抑郁障碍的前额叶皮质、扣带回、杏仁核、岛叶等脑区功能紊乱的差异,提示两种疾病神经生物机制的不同病理改变,但两种疾病的生物标记仍然不清楚.在未来的研究中,进一步加深对单、双相抑郁障碍病理机制、神经可塑机制的理解,可以更好地促进临床鉴别诊断的准确性、抗抑郁药物疗效与安全性的提高.

Unipolar and bipolar depression both are characterized by low mood, low interests and anhedonia. It is diffi-cult to distinguish bipolar depression from unipolar depression in clinic. In this study, we reviewed the literatures related with the neuopahological changes in both mental disorders. It showed that unipolar depression and bipolar depression had different functional disruptions in prefrontal, cingulate gyrus, amygdala, and insular lobe reflected by the functional magnet-ic resonance imaging studies. However, so far there have been no consensuses on the different neuropsychopathological mechanisms between the unipolar and bipolar depression. In the future studies, further understanding of pathological mech-anisms and neural plasticity of unipolar and bipolar depression can help to improve the diagnostic accuracy, antidepressant treatment and safety of medicine.
