面向儿童青少年的创伤聚焦的认知行为治疗:干预模型与实践启示

摘要:

认知行为治疗是-种结构化的、有效的心理干预方法,在创伤后心理干预领域得到了广泛应用,且在发展过程中开始聚焦于创伤事件。本文重点介绍了面向儿童青少年的创伤聚焦的认知行为治疗(Trauma-focused Cognitive-behavioral Therapy,TF-CBT)的干预模型,包括心理教育与亲职训练、放松训练、情绪调控训练、认知应对训练、创伤复述与对创伤经验的认知加工、创伤线索暴露、亲子联合治疗、促进安全与未来发展轨迹等八个步骤和要素。在创伤后心理干预领域,这种方法相对于非指导性心理治疗、心理动力治疗、催眠疗法和压力管理等方法,对缓解创伤后应激障碍、焦虑和抑郁等症状有更显著的效果。本文在回顾前人研究的基础上,提出了对我国创伤后心理干预研究与实践的建议。

Cognitive-behavior therapy(CBT) is a structural and effective approaches for post-trauma intervention.Recently, focusing on the trauma itself has been highlighted in this psycho-theropy.This article aimed to introduce the intervention model of trauma-focused cognitive-behavioral therapy(TFCBT), consisting with eight elements, such as psycho-education, and taining on parenting skills, relaxation skills, affective modulation skills and cognitive coping skills, trauma narrative and cognitive processing trauma experiences, in vivo mastery of trauma reminders, conjoint parent-child sessions, enhancing safety and future development trajectory.In the field of posttraumatic psychological intervention, TFCBT is more effective than nondirective therapy, psychodynamic therapy, hypnotherapy and stress management on the post-trauma stress disorder, anxiety and depressive symptoms.This study reviewed the works of former researchers and provided advice for future research and intervention practice in China.
